Chronic Condition Management

Upcoming Changes to Enhance Chronic Condition Management

Changes to the Medicare Benefits Schedule items for chronic disease management have been deferred from 1 November 2024 until 1 July 2025. The additional time will support all practices and providers, including GPs and allied health providers, to be ready for the changes to these important services.

Chronic diseases are a leading cause of illness and death in Australia, and with an ageing population, these changes aim to improve care delivery. The changes aim to:

simplify, streamline, and modernise the arrangements for health care professionals and patients
promote continuity of care
encourage the regular review of chronic condition management plans
support communications between a patient’s multidisciplinary care team
ensure existing patients can continue to access the care they need.

What can your practice do to prepare?

Consider these steps to get your practice team ready for the upcoming changes

Discuss with Your Team: Review what these changes mean for your practice. Identify activities or processes that may need updating, such as incorporating MyMedicare registration into your chronic disease/conditions management workflows.
Register Your Practice: If you haven’t already, register your practice for MyMedicare.
Register Your Patients: Prioritise registering patients with chronic conditions or existing care plans in MyMedicare. Primary Sense helps identify patients who are at risk or experiencing fragmented care within a practice. The Voluntary Patient Registration report highlights these at-risk patients who would benefit from enrolment. This report aims to enhance continuity of care, strengthen the relationship between patients and their General Practice, and help practices better meet patient needs. By capturing data on patients who may qualify for voluntary registration, the report enables practices to initiate these important conversations at the point of care.
Engage Your Patients: Encourage patients, especially those with chronic conditions or existing care plans, to register with MyMedicare:
1. Team Preparation: Ensure your practice team is prepared to discuss MyMedicare with patients. Hold a practice meeting or provide them with MyMedicare brochures and frequently asked questions to support patient conversations.
2. Patient Forms: Include the MyMedicare Registration form and brochure with your patient consent and data collection forms.
3. Website Update: Add links to MyMedicare brochures, patient registration information, and frequently asked questions on your practice’s website.
4. In-Practice Displays: Place MyMedicare posters and brochures in your waiting room/reception area and display MyMedicare patient videos on your waiting room screens.
5. Social Media: Share information about MyMedicare on your practice’s Facebook or social media pages using the available social media tiles.

The MyMedicare General Practice toolkit include social media tiles, videos, brochures, and posters to assist you in engaging your patients.
