Chronic Disease Management (CDM) changes are planned for 2025. This will be the first major change to CDM in 20 years and is proposed to come into effect on 1 July 2025.
With a quarter of revenue coming from CDM, commencing steps to prepare your practice for the transition offers your team an opportunity to reduce future workload, and ensure revenue remains consistent and care to patients is unaffected. This will reduce the burden of change, improve data quality, and maintain accurate and up-to-date-systems, processes and policies for a streamlined transition.
GCPHN’s Digital Health and Engagement Team are here to support you every step of the way. We will be booking practice visits over the coming weeks to assist with Primary Sense training, data cleansing activities and the first in a series of self-paced and supported quality improvement activities. These activities aim to identify patients with high to moderate complexity characterised by multimorbidity, instability and polypharmacy (ACG 5 &4) who would benefit from MyMedicare Registration.
