The Way Back Support Service (TWBSS) Out of Hospital (OOH) pathway is a low to high intensity non-clinical psychosocial support and transition service for people experiencing suicidality and have presented to either a General Practitioner (GP), Head to Health (H2H) clinician or Aboriginal Medical Service (AMS).
Personalised non-clinical psychosocial support for up to three months. This service supports people to stay safe, keep connected with others, and access health and community services as part of their recovery.
Suicide Prevention
A Support Facilitator contacts the person within one business day of receiving their referral and tailors a program for up to three months which includes building a personalised safety plan and linking them to health and community services and clinical care if needed. This program sits within the suite of Wesley Mission Queensland services and can link into psychological supports if required.
People residing in Gold Coast aged 16 years and older that have presented to a General Practice, Head to Health phone line or Aboriginal Medical Service.
