The GCPHN Quality Improvement (QI) team supports the primary healthcare workforce to successfully undertake continuous quality improvement (CQI). The aim is to support general practice to deliver integrated, person-centered care and to improve quality and safety for patients through areas such as practice structures, systems, processes and clinical care.
Undertaking quality improvement activities allows a practice to move from a reactive model of care to a model that supports patients to self-manage, stay well and engage in regular preventative care.
